Parents, students, and teachers across Illinois have an opportunity to participate in the fourteenth annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey. These elements include:
To take the survey please visit https://survey.5-essentials.org/illinois/survey/parent/login/ and select the appropriate survey to begin. If you have a student at more than one school, please fill out a survey for each school. SEG is listed as "S E Gross Middle School" and the spaces between these letters are required when searching for (and finding!) our school. Brook Park is "Brook Park Elem School". Your participation in the parent portion of the survey will help us understand your family’s experience and guide our planning and priorities. Please know your identity and survey responses will be kept completely confidential and will never be connected to you or your child. Each school needs at least 20% parent participation to generate a detailed report that can be used for school improvement planning. Right now, only 11% of families have completed this survey.
