Free and Reduced Application

District 95 Instructions for Free/Reduced Lunch Application/Fee Waiver 

If you feel you may qualify for the Free/reduced lunch program, print your application, complete, sign and return the application with the qualifying document(s) listed below to the school office. 

ONE of the qualifying documents must accompany the signed application: 

• SNAP, TANF, or Medicaid correspondence with current dates of eligibility 

• Copy of most current tax form 

• 2 most recent pay stubs from all income sources in the household 

• Unemployment verification and/or recent letter of unemployment from the employer 

To have your student begin receiving free/reduced lunch on the first day of school, applications must be submitted by July 15, 2024. Applications can be submitted throughout the year but will be subject to possible fee proration or require additional documentation. All applications and original qualifying documents need to be dropped off at your school office. Incomplete applications will not be accepted.
